(a) Upon the receipt of the required application, the division shall issue a certificate to the applicant.
(b) The certificate shall contain a unique number that corresponds to the individual named on the certificate.
(c) Certificates shall be valid for the lifetime of the holder.
(d) Certificates shall not be transferable.
