Each applicant for a chair van company license shall complete and submit a " New Hampshire Bureau of EMS Wheelchair Van-For-Hire Company Application" form, as revised 7/18/16, available at https://www.nh.gov/safety/divisions/fstems/ems/documents/wheelcompany.pdf.
