Each applicant for an EMS I/C license shall complete and submit "New Hampshire Bureau of EMS EMS Instructor/Coordinator License Application" form, as revised 1/07/19, available at https://www.nh.gov/safety/divisions/fstems/ems/documents/a29.pdf.
