(a) Upon submitting the application as set forth above, the owner shall furnish positive proof of identification to the department.

(b) Proof shall include one or more of the following:

(1) Current driver license or driver license which has expired no more than 12 months prior to the date of application;

(2) Current passport;

(3) Military discharge papers;

(4) Current non-driver identification card or non-driver identification card which has expired no more than 12 months prior to the date of application;

(5) A birth certificate;

(6) Military identification card;

(7) A government issued identification;

(8) An employee photo identification card;

(9) Permanent resident card; or

(10) Naturalization certificate.
