(a) A registrant applying for a replacement registration plate shall furnish the following on the prescribed form:
(1) Name and
address;
(2) Certification that the
plate issued has been lost, stolen, damaged, or destroyed;
(3) Whether the loss has been reported to a
local police department and the name of such department;
(4) The registration plate number and type;
and
(5) Signature of the
registrant.
(b) The application form shall be submitted, in person to a substation or authorized municipal agent or, in person or by mail, to:
