Section Saf-C 414.02 - Water Event or Slalom Course Permit Application

(a) An applicant for a permit to conduct a water event or slalom course shall provide the commissioner with the following:

(1) Name, address, date of birth, telephone number and cellular telephone number, if available, of the applicant;

(2) The type of water event;

(3) The name of the water event, if applicable;

(4) The name of the public water on which the water event will be conducted;

(5) The name of the town or city where the public water is located;

(6) The date(s) the water event will be conducted;

(7) The start and end time of the water event each day it is conducted;

(8) A brief description of the water event;

(9) The number of participants in the water event;

(10) A map or sketch of the water event area, including the direction "north" as a reference; and

(11) Signature of the applicant and date signed.
