(a) Upon approval of a mooring permit application, the division shall issue a mooring permit to the applicant.

(b) The permit shall include:

(1) The address of the shorefront property where the mooring(s) are located;

(2) The date the mooring permit was approved;

(3) A statement that the mooring permit shall not be transferred to any other person location by gift, sale, lease or rent; and

(4) Any conditions attached to the mooring permit, such as but not limited to:
a. Whether orange buoys with non-strobe flashing orange lights shall be required to establish the perimeter of the mooring field;

b. The maximum number of moorings allowed at the specified shorefront property;

c. That boats be required to be tied bow and stern;

d. The removal of a line extending from the dock to the mooring;

e. A final court ruling, if litigation occurred regarding the mooring permit;

f. The specific location requirements of the placement of the mooring(s); and

g. The maximum boat size that may be attached to the mooring.
