(a) Each applicant for a mooring permit pursuant to RSA 270:61 shall complete:
(1) Form DSSS 18, application for an
individual mooring permit, if applying for one to 4 moorings;
(2) Form DSSS 19, application for a
congregate mooring permit, if applying for a congregate mooring field that is 5
or more moorings; and
(3) Form DSSS
33, application for a public mooring field permit, if applying for a public
mooring field, that is 5 or more moorings.
