(a) Any person who has received training in another jurisdiction may comply with the provisions of Saf-C 3904 by:

(1) Providing proof of successful course completion, as set forth in (b) below; or

(2) Having certification that has been accredited through the National Board on Fire Service Professional Qualification in Quincy, Massachusetts, or the International Fire Service Accreditation Congress in Stillwater, Oklahoma.

(b) Proof of successful course completion such as a course certificate, shall include the following:

(1) Date of completion;

(2) Name of level of competency obtained;

(3) Name of instructor and training agency;

(4) Name of person certified;

(5) Number of practical and classroom instructional hours;

(6) A statement that the course meets or exceeds the competencies established by NFPA 472 Standard for Professional Competence of Responders to Hazardous Materials Incidents, 2008 edition; and

(7) Signature of training provider.

(c) A statement may be obtained from the training agency and provided separately in lieu of (b)(5) or (6), above.

(d) In the event that the training that forms the basis of a reciprocity request pursuant to (a), above was obtained in excess of 2 years prior to the request, the person shall be required to pass the final examination for the level of competency applied for, as offered by the division.
