Section Saf-C 3119.03 - Application for Renewal of CDL Driver Education Standard Certificate

(a) Each applicant for the renewal of a CDL driver education standard certificate shall complete and submit the form DSMV 635, "Application for Driver Education Instructor Certificate" (rev 02/18).
(b) The applicant shall provide a certified copy of his or her motor vehicle record from any state in which the applicant has held a driver license and from any other state in which the applicant has obtained a motor vehicle conviction since his or her initial application.
(c) The applicant shall provide a certified copy of his or her criminal record from this state and from any other state in which the applicant has obtained a criminal conviction since his or her initial application.
(d) The applicant shall sign and date the form under penalty of unsworn falsification.
(e) Pursuant to RSA 21-P:14, I(f), a fee of $40 shall be submitted along with the application.
