Section Saf-C 3119.02 - Application for CDL Driver Education Certificate

(a) Each applicant for a CDL driver education instructor certificate shall complete and submit the form DSMV 635, "Application for Driver Education Instructor Certificate" (rev 02/18).

(b) The applicant shall submit an original certificate of completion of a driver training program compliant with 49 CFR 380.

(c) The applicant shall provide a certified copy of his or her motor vehicle record from any state in which the applicant has held a driver license and from any other state in which the applicant has obtained a motor vehicle conviction.

(d) Pursuant to RSA 263:44-b, every applicant for an initial CDL driver education certificate shall execute a criminal history record release form authorizing the release of his or her criminal history record to the division. The applicant shall also submit a complete set of fingerprints taken by a qualified law enforcement agency or authorized employee of the department.

(e) The applicant shall sign and date the form under the penalty of unsworn falsification.

(f) If requesting a driver education provisional certificate, a fee of $25 shall be submitted along with the application.

(g) If requesting a driver education standard certificate, a fee of $40 shall be submitted along with the application.
