(a) Application forms may be obtained from:
Department of Safety
Division of Motor vehicles
Driver Education Section
23 Hazen Drive, Concord, NH 03305.
(b) A completed application shall include the following:
(1) Proof of driving experience as
set forth in
Saf-C
3103.02;
(2) Report of physical examination as set
forth in
Saf-C
3103.03;
(3) Successful completion of written and
on-road examinations as set forth in
Saf-C
3103.04;
(4) Criminal history record check;
(5) Fingerprints, upon initial application
only, pursuant to
RSA
263:44-b; and
(6) Motor vehicle record from any state the
applicant has held a driver license.
(c) Completed application forms shall be submitted to the:
Department of Safety
Division of Motor Vehicles
Driver Education Section
