(a) An applicant for a renewal license for security guard agency or private investigative agency shall complete and submit form DSSP 221 "Agency Application for Renewal of Private Investigative, Security Guard, or Bail Recovery Agency" License revised 3-20.

(b) The person making application for the agency shall provide a detailed explanation on form DSSP 221 if the person answers affirmatively as to whether the person making application for the agency has been arrested.

(c) The applicant shall sign and acknowledge that he or she certifies that he or she has read the application and affirms that every statement contained herein is true and correctly set forth and he or she also certifies that he or she is familiar with all state laws, rules and local ordinances relating to the license for which he or she is applying, for the locations in which he or she intends to conduct operations.
