(a) Each request for a hearing shall be in writing and contain the following information:
(1) Name, address and telephone
number of person requesting the hearing;
(2) Make and registration of vehicle in
question;
(3) Date and time vehicle
was removed from its location;
(4)
Location of vehicle after removal;
(5) Name of authorized officer involved, if
known; and
(6) Reason for
contesting the removal.
(b) All requests for hearings shall be addressed to:
