(a) Any person wishing to establish an approved driver attitude program shall submit to the commissioner an application containing the following information:
(1) The name, address and telephone number of
the organization and any affiliation or license necessary to satisfy
Saf-C
212.10;
(2) A complete description of the program as
outlined in Saf-C
212.10(a);
(3) A list of all instructors who shall be
teaching as part of the program, including their names, dates of birth,
addresses, educational background, and work experience; and
(4) A complete description of the record
system that the program will use to satisfy
Saf-C
212.10(a)(7).
