(a) In the event a person changes his or her name, he or she shall furnish the following on form DSMV 30 "Record Change Request" (rev. 3/22):

(1) Former name;

(2) New name;

(3) Date of birth; and

(4) Signature.

(b) As evidence of a new name, a person shall present a copy of one of the following:

(1) Name change petition from the court of probate;

(2) Marriage certificate;

(3) Divorce decree;

(4) Passport;

(5) Adoption decree; or

(6) Court decree.

(c) In the event a corporation, partnership, or other legal entity changes its name, it shall be presumed that the change constitutes a transfer of ownership, unless satisfactory proof to the contrary is furnished, such as a certificate from the secretary of state's office showing the name was changed and that no corporate officers have changed.
