(a) The department shall furnish application form DSMV 312 at any substation or the:
Division of Motor Vehicles
23 Hazen Drive
Concord, NH 03305
(b) Each applicant shall complete and submit form DSMV 312, in person to a division substation.
(c) Applicants shall submit form DSMV 312 to a division substation for each of the following:
(1) Original commercial driver
license;
(2) Renewal commercial
driver license;
(3) Duplicate
commercial driver license; and
(4)
Replacement commercial driver license.
