(a) Each registrant requesting a temporary registration authorization shall complete an application.

(b) Each registrant shall supply on or with the application for temporary registration authorization the following:

(1) Applicant's name;

(2) IRP account number and equipment number;

(3) Owner's name;

(4) Mailing address;

(5) Vehicle identification number;

(6) Vehicle type, year, make and gross weight;

(7) Number of axles for each truck or tractor combination, or number of seats for each bus;

(8) Fuel type;

(9) Current plate number;

(10) US DOT number;

(11) Jurisdictions and weights to be registered;

(12) Certification that the information is true and that the temporary registration authorization will be used in accordance with applicable statutes and rules;

(13) Signature of applicant and date signed;

(14) Beginning and ending dates for which the temporary registration authorization is requested;

(15) A statement that the temporary registration authorization shall be valid for 45 days from the date of issuance, pending permanent registration; and

(16) Acknowledgement that an application for a permanent apportioned registration shall be filed within 10 days of issuance of the temporary registration authorization or the registrant's privilege to operate in New Hampshire shall be suspended.

(c) A temporary registration authorization shall also be available from permit service companies.

(d) A temporary registration authorization shall allow a New Hampshire based fleet vehicle to operate in the listed jurisdictions and at the listed weights for a period of 45 days pending issuance of permanent IRP registration.
