(a) Each IRP applicant shall supply on or with schedule F the following:

(1) The IRP account number assigned when the original application was filed;

(2) The fleet number, if different fleets are registered under the same company name;

(3) The supplement number, starting consecutively with 01;

(4) The last 2 digits of the registration year;

(5) Name of the person, firm, or corporation having a New Hampshire apportioned registration;

(6) Business address in New Hampshire;

(7) Mailing address, to where all correspondence will be sent;

(8) Name address and telephone number of person to be contacted to answer questions pertaining to the application;

(9) Type of fleet operation;

(10) Equipment number, uniquely assigned to each vehicle in the fleet;

(11) Plate number for each vehicle;

(12) IRP member jurisdiction to be added to the cab cards of a fleet already apportioned under the IRP;

(13) Maximum weight to be carried in the added jurisdiction by the listed vehicle;

(14) An estimate of the total fleet miles to be traveled in the added jurisdiction;

(15) A brief statement explaining why the jurisdiction is to be added to the fleet territory, including a revised mileage estimate based upon travel in the added jurisdiction;

(16) Operating authority issued by the FMCSA, if required by the jurisdiction to be added;

(17) US DOT number;

(18) Taxpayer identification number; and

(19) Signature of person authorized to add a jurisdiction to the fleet, and date signed.

(b) For the purposes of (a)(6) above, a post office box shall not be acceptable.
