(a) Each applicant shall complete an original application on schedule A/E and schedule B.

(b) Each applicant shall supply on or with schedule A/E the following:

(1) Name of the person, firm or corporation requesting a New Hampshire apportioned registration;

(2) Business address;

(3) Mailing address, to where all correspondence will be sent;

(4) Name, address and telephone number of person to be contacted to answer questions pertaining to the application;

(5) Whether the operation is an exempt commodity carrier, household goods carrier, private carrier, for hire carrier, rental company or whether the fleet is leased to a private carrier or for hire carrier;

(6) Maximum weights to be carried in each jurisdiction in which the fleet will be apportioned;

(7) The following information for each vehicle to be registered:
a. Equipment number;

b. Manufacturer's model, year and make;

c. Complete vehicle identification number (VIN);

d. Vehicle type, using the abbreviations listed at the top of schedule A/E;

e. Number of axles for each truck or tractor combination, or number of seats for each bus;

f. Unladen weight;

g. Fuel type, using the abbreviations listed at the top of schedule A/E;

h. Combined or gross weight, meaning the unladen weight of the vehicle plus the weight of the load;

i. Actual purchase price;

j. Manufacturer's list price of the full equipped vehicle when new;

k. Date of purchase, if purchased;

l. Date of lease, if leased;

m. Name and signature of owner, if other than the applicant;

n. Number of current title certificate;

o. Horsepower, if the vehicle is a bus;

p. Abbreviation of title state, using 2 digits;

q. Current license plate number and state of registration;

r. United States Department of Transportation (US DOT) number; and

s. Taxpayer identification number;

(8) Total of:
a. Power units in the fleet;

b. Trailers in the fleet; and

c. Vehicles in the fleet;

(9) Operating authority issued by the Federal Motor Carrier Safety Administration (FMCSA), to the jurisdictions requested, if applicable; and

(10) Signature of person authorized to apply for the apportioned registration and date signed.

(c) For the purposes of (b)(2) above and (g)(2) below, a post office box shall not be acceptable.

(d) For the purposes of (b)(6) above, separate pages shall be used if the weights in all jurisdictions are different for each vehicle.

(e) For the purposes of (b)(8) above, an applicant shall assign a unique number or letter to each vehicle in the fleet.

(f) For the purposes of (b)(7)p. above, if a vehicle has not been previously registered in New Hampshire, the applicant shall submit a copy of the title certificate from the state of current registration. If a vehicle is titled in a state other than the state of registration or if a vehicle is not yet titled, the applicant shall submit an application for a New Hampshire title certificate.

(g) Each applicant shall supply on or with schedule B the following:

(1) Name of person, firm or corporation requesting a New Hampshire apportioned registration;

(2) Business address;

(3) Mailing address, to where all correspondence will be sent;

(4) The reporting period;

(5) Name, address and telephone number of person to be contacted to answer questions pertaining to the application;

(6) Whether the operation is an exempt commodity carrier, household goods carrier, private carrier, for hire carrier, rental company or whether the fleet is leased to a private carrier or for hire carrier;

(7) Mileage in each jurisdiction in which the fleet traveled during the year preceding the current registration year being applied for;

(8) An "X" shall be placed beside each IRP member jurisdiction in which the applicant wishes to have an apportioned registration;

(9) An explanation of any mileage estimated; and

(10) Signature of person authorized to apply for the apportioned registration and date signed.

(h) For the purposes of (g)(7) above, the following shall apply:

(1) If no reportable mileage for the previous year is available for a jurisdiction that will be traveled through, an estimate of miles to be traveled shall be entered;

(2) Mileage shall be entered whether the jurisdiction is an IRP member or not; and

(3) Mileage shall be entered separately for each jurisdiction.

(i) Each applicant shall have an established place of business which is registered with the secretary of state's office in New Hampshire.

(j) Each applicant shall ensure that records are maintained and available for audit.
