(a) A holder of a firm certificate of registration shall notify the department, in writing, by mail, to the address set forth in Saf-C 1203.02(a), within 30 calendar days of the following:
(1) A change in any of the
officers set forth in
Saf-C
1203.02(b)(6);
(2) A change in the business name or
address;
(3) A change in the RME at
the firm's location;
(4) The
business ceases to operate; or
(5)
The business ceases to operate under the name listed on the
certificate.
