Notice of intent to retire under the provisions of RSA 100-A:5 or RSA 100-A:10 shall be made by completing and filing the group I or group II member application for service retirement, or under the provisions of RSA 100-A:6 by completing and filing the group I or group II member application for ordinary and/or accidental disability retirement. Except in the case of an application for a vested deferred retirement benefit, such forms, as identified in Ret 305.05, shall be filed with the board of trustees not less than 30 nor more than 90 days prior to the requested effective date of retirement.
