All petitions, motions, memoranda, briefs, affidavits, and similar documents filed pursuant to this chapter shall:

(a) Be clearly printed on 8-1/2" x 11" size paper, which is hand-delivered, mailed, or be sent electronically by fax or as an email attachment;

(b) Contain the title of the proceeding, if known;

(c) Be dated, with the original signed; and

(d) State the title, if any, and address of the signer.
