Section Resp 403.02 - Application Procedures For Reinstatement of Suspended or Lapsed Licenses

All applicants for reinstatement of licensure shall apply by:
(a) Submitting to the board a completed reinstatement application form dated June 25, 2015;
(b) Paying the reinstatement fee set forth in Ahp 301.02(a); and
(c) Submitting, or arranging for the submission of, the following:
(1) An original
recent passport size, 2" x 2", head-shot photograph;
(2) A detailed report of the relevant
circumstances if any of the answers to the starred questions on the
reinstatement application form are in the affirmative;
(3) A written statement that the
reinstatement applicant has not engaged in respiratory care in New Hampshire on
a volunteer or paid basis since the date that his or her license ceased to be
valid;
(4) If applying for
reinstatement of a lapsed license pursuant to
Resp
404.01 through
Resp
404.05, a current, original criminal offender record
report from each state where the applicant has been licensed or has resided
within the past 6 years, provided that such state(s) will release such
report(s):
a. Dated within the 6 months
preceding the application for licensure; and
b. Sent:
1.
Directly to the board; or
2. To the
applicant to be forwarded to the board;
(5) Transcripts as described in
Resp
302.04(e), if not previously
provided; and
(6) Any additional
materials specified for the applicable license reinstatement category from the
list of reinstatement categories set forth in Resp 404.
