Section Resp 402.01 - Procedure for Licensees Who Do Not Wish to Renew Their Licenses

Any licensee who does not wish to renew his or her license shall so indicate by:
(a) Submitting only the first page of the renewal form on or before December first of the renewal year; and
(b) Completing that page only to the following extent:
(1)
Correcting or adding as necessary to the preprinted home mailing address:
and
(2) Placing on the lines
provided for a licensee not wishing to renew:
a.His or her signature; and
b.The date of signing.
