(a) A business applying for a grant shall supply on Form SJG-1 the following information:

(1) Name, address and telephone number of the business applying;

(2) Name and title of the contact person;

(3) Type of business;

(4) Description of the business ownership, and the business organization, both internally and as part of a larger corporate structure, if applicable;

(5) Description of the existing positions and the positions to be created by title, compensation, and benefits to be provided;

(6) Description of the hiring requirements for those positions; and

(7) An indication of how many of the new jobs created will be filled by people from low and moderate income families.

(b) All information shall be typewritten.

(c) The business shall include a copy of the following:

(1) Certificate of existence from the Secretary of States office;

(2) Corporate resolution giving authority to the contact person to sign documents on behalf of the business;

(3) A business plan for at least the next 5 years, or other evidence of the ability to employ people for the required 5 year period;

(4) Audited, consolidated, or descriptive financial statements for at least the last 3 years.

(d) The business shall agree to report any portion of the grant that will not be used as required by Res 2607.03.

(e) The contact person representing the business shall:

(1) Sign and date the application; and

(2) Certify by his or her signature to guarantee the jobs created pursuant to Res 2603.02.

(f) The applicant shall mail or hand-deliver the original and 2 copies of the application and all attachments to:
