(a) A local governing body requesting CROP zone designation shall supply on Form CPZ-1 the following information:

(1) Name, address and telephone number of the local governing body;

(2) Name and title of the contact person; and

(3) Name of area or site to be designated a CROP zone.

(b) All information shall be typewritten.

(c) The local governing body shall provide evidence of the following:

(1) The area has been certified as being a brownfields site; or

(2) The area has at least one of the following characteristics:
a. The population of the municipality or municipalities in which the zone is located shows a decrease, according to the most recent federal census;

b. At least 51 percent of the households in the census tract or tracts in which the zone is located have incomes of less than 80 percent of the median income of households in the state, as reported by the United States Department of Housing and Urban Development and the New Hampshire department of employment security, bureau of economic and labor market information;

c. At least 20 percent of households in the census tract or tracts in which the zone is located have a median income below the federal poverty level, as reported by the United States Department of Housing and Urban Development and the New Hampshire department of employment security, bureau of economic and labor market information; or

d. The zone contains either:
1. Unused or underutilized industrial parks; or

2. Vacant land, or structures previously used for industrial, commercial, or retail purposes but currently not so used due to demolition, age, obsolescence, deterioration, relocation of the former occupants operations, or cessation of operation resulting from unfavorable economic conditions either generally or in a specific economic sector; and

(3) Designation of the zone as a CROP zone would likely result in the reduction of the rate of vacant or demolished structures or the rate of tax delinquency in the zone.

(d) The contact person representing the local governing body shall sign and date the form.

(e) The local governing body shall mail or hand-deliver the completed form to:
