Section Res 2404.01 - Form EDP-2, Economic Development Matching Grants Application

(a) A municipality, county or organization applying for a grant shall supply on Form EDP-2 the following information:

(1) Name, address and telephone number of the municipality, county, or organization;

(2) Name and title of the contact person;

(3) Name and title of the person responsible for billing;

(4) Project title;

(5) Project starting date and project completion date as specified in Res 2403.01;

(6) Project budget, including:
a. Total funds raised;

b. Matching funds requested; and

c. Total project cost;

(7) Statement of the proposed project's objectives;

(8) Detailed description of the project;

(9) Detailed statement of the funds that have been raised or will be raised for the proposed project; and

(10) Detailed budget of all expenses, matchable and unmatchable.

(b) All information shall be typewritten.

(c) The municipality, county, or organization shall include a copy of the following:

(1) Ad samples, if applicable;

(2) Brochure dummy, if applicable;

(3) At least 3 competitive bids for printed materials;

(4) Media or distribution schedule, if applicable;

(5) Television story board and script, if applicable;

(6) Radio script, if applicable; and

(7) Mock-ups, if other materials are to be produced.

(d) The applicant shall agree to:

(1) Attend the meeting of the screening committee and give a presentation on the proposed project as required by Res 2403.10;

(2) Prominently display on all promotion projects:
a. The words "New Hampshire"; and

b. The state logo provided by the division of economic development (DED) and used in its advertising campaigns;

(3) Submit all bills for reimbursement as required by Res 2408.02;

(4) Report any portion of the grant that will not be used by the completion date as required by Res 2408.04;

(5) Submit to a project audit, if requested, as part of an audit of the DED; and

(6) Submit a final report after completion of the project as required by Res 2409.

(e) The applicant and 2 officers of the municipality, county, or organization shall sign and date the application.

(f) The applicant shall mail or hand-deliver the original and 8 copies of the application and all attachments to:
