Current through Register No. 13, March 27, 2025

(a) The applicant shall complete and submit the "Universal Application for Initial License" dated March 2023 signed and dated in accordance with Plc 304.05;

(b) In addition to the application in (a) above, the applicant shall provide a list of all names they have ever been known by;
