(a) Each applicant shall complete an application, form PEC 900, to participate under RSA 188-D:18, I, in the workforce incentive program.
(b) The applicant shall supply the following information on form PEC 900:
(1) His or her first name and last
name;
(2) Applicant's
current address;
(3)
Applicant's current employer;
(4) Employer's address;
(5) Beginning and ending date of
service for which loan repayment is sought;
(6) Name and title of certifying
official;
(7) Signature
of certifying official; and
(8) Educational loan information as
follows:
a.Date, amount and
interest rate of each educational loan;
b.Balance of each educational
loan;
c.Name of servicer
of each educational loan;
d.Address of servicer of each
educational loan.
(c) The application shall be signed and dated by the applicant.
(d) The applicant shall attach a copy of the most recent loan payment invoice(s).
