The commission shall cancel a recipient's repayment obligation if it determines that:
(a) The recipient is unable to work due to a permanent total disability, established by the sworn affidavit of a physician;
(b) The recipient has received relief under the federal bankruptcy laws only if the recipient's loans under Title IV of the Higher Education Act of 1965, 20 U.S.C. 1070 et seq., as amended have been canceled as a result of that bankruptcy; or
(c) The recipient has died, established by a death certificate or other evidence deemed conclusive under state law.
