(a) Each applicant for a tuition waiver under the tuition waivers for foster children program shall make application on Form PEC-FC, application for scholarship for foster children, by supplying the following information:
(1) Applicant's first
and last name and middle initial;
(2) Applicant's mailing
address;
(3) Applicant's
present legal residence;
(4) Applicant's telephone number
and e-mail address;
(5)
Applicant's date of birth and final four digits of his/her social
security number;
(6)
Enrollment information including:
a.College or school that the
applicant plans to attend or is attending; and
b.Current grade level;
(7) Name and address of
living parent or guardian, or other personal contact who knows where
you may be reached;
(8)
Date on which the Free Application for Federal Student Aid (FAFSA)
was submitted;
(9) Date
on which the application for aid under the New Hampshire charitable
foundation's Statewide Student Aid Program or Career Aid to
Technology Students (CATS) Program was submitted;
(10) Signed statement from the
division of children, youth and families certifying applicant's
eligibility status.
(b) Each applicant shall sign and date the application form.
(c) The notarized signature shall certify that all information provided on the application is truthful.
