(a) The school shall submit form PEC 1100-10 to the director for its review and approval; and
(b) The program information shall include, but not be limited to:
(1) Program description;
(2) Resources required to implement
and maintain the program, such as personnel, materials, equipment,
finances, and facilities;
(3) Resumes of instructors;
and
(4) The entity shall
submit additional information, as required, to enable the director to
make a substantiated and informed decision.
