A complaint shall include:
(a) Identification of the profession to which the conduct being complained of relates;
(b) The name of the individual or business that is alleged to have engaged in the conduct complained of;
(c) If known, the type and license number of the license held by such individual or business, if any;
(d) A clear and concise statement of the facts on which the complaint is based, including but not limited to the following for each occurrence of the conduct being complained of:
(1) A description of the specific conduct
that forms the basis of the complaint;
(2) The date and time the conduct occurred,
provided that if the conduct occurred on more than one occasion, the date and
time of the most recent occurrence may be provided with a statement of the
overall time frame in which the conduct occurred and the number of times the
conduct was repeated;
(3) The
location or locations where the conduct occurred; and
(4) Whether there were any other individuals
present when any instance of the conduct occurred and, if known, the name of
and contact information for each such witness or observer;
(e) Information about the individual who is making the complaint, including:
(1) The
complainant's first and last name;
(2) The telephone number including area code
and extension, if any, at which the complainant can be reached during normal
daytime business hours; and
(3) The
complainant's email address and mailing address; and
(f) Whether the complainant has:
(1) Attempted to resolve the complaint with
the licensee;
(2) Retained an
attorney in the matter, and if so the name, address, email address, and
telephone number including area code of the attorney; and
(3) Reported the complaint to any other
local, state, or federal agency and if so, the agency's name and the name,
email address, and telephone number of a contact to whom the complaint was
made, if known.
