Section Phy 404.04 - Supervised Practice

(a) Licensees holding conditionally reinstated licenses shall practice under supervision as described in Phy 404.04 for a total of 320 hours.
(b) The levels of supervision shall be:
(1) For those who have
not been active in the profession for 4 to 6 years:
a. The first 200 hours shall be:
1. Direct personal supervision, as defined in
Phy
401.04, for at least 50% of the hours worked each
week;
2. Direct supervision, as
defined in Phy 401.05, for at least 40% of
the hours worked each week; and
3.
General supervision, as defined in
Phy
401.07, for no more than 10% of the hours worked each
week; and
b. The 120
hours immediately following the first 200 hours described in a. above shall be:
1. Direct personal supervision, as defined in
Phy
401.04, for at least 30% of the hours worked each
week;
2. Direct supervision, as
defined in Phy 401.05, for at least 30% of
the hours worked each week; and
3.
General supervision, as defined in
Phy
401.07, for no more than 40% of the hours worked each
week; and
(2)
For those who have not been active in the profession for more than 6 years:
a. The first 200 hours shall be:
1. Direct personal supervision, as defined in
Phy
401.04, for at least 70% of the hours worked each
week;
2. Direct supervision, as
defined in Phy 401.05, for at least 20% of
the hours worked each week; and
3.
General supervision, as defined in
Phy
401.07, for no more than 10% of the hours worked each
week;
b. The 120 hours
immediately following the first 200 hours described in a. above shall be:
1. Direct personal supervision, as defined in
Phy
401.04, for at least 30% of the hours worked each
week;
2. Direct supervision, as
defined in Phy 401.05, for at least 30% of
the hours worked each week; and
3.
General supervision, as defined in
Phy
401.07, for no more than 40% of the hours worked each
week.
