Part Phy 402 - PROCEDURES FOR RENEWING A LICENSE AND INDICATING A DECISION NOT TO RENEW
Section Phy 402.12 - Full Renewal of Licensure

(a) The board shall issue fully renewed licenses to applicants who:
(1) Fall into one of the following 3
categories:
a. Applying for licensure renewal
for the first time;
b. Applying for
licensure renewal no more than 4 years after issuance of the initial license;
or
c. Applying for licensure
renewal on any date more than 4 years after issuance of the initial license if
the applicants have been active in the profession as defined in
Phy
401.01 for the 4 years just preceding their
applications; and
(2)
Meet the basic application requirements of (b) below.
(b) The basic application requirements shall be:
(1) Maintenance of continuing competence
as further described in Phy 406;
(2) Having not:
a. Violated RSA 328-A;
b. Violated RSA 328-F; or
c. Demonstrated poor moral character as
evidenced by:
1. The answers to the questions
in Phy
402.04(e)(2) and
Phy
402.04(k); and
2. Any related documents submitted pursuant
to Phy
402.07(a) or (b); and
(3) Having timely
submitted the completed renewal application packet;
(4) Have taken and passed the jurisprudence
examination described in
Phy
402.06.
