(a) The revocation of a pharmacist license shall permanently withdraw the authority to practice pharmacy in New Hampshire.
(b) A subsequent license may be obtained only by:
(1) Complying with
all of the requirements of RSA 318 and these rules regarding the original
licensing of pharmacists;
(2)
Paying all penalties assessed in connection with the cause for revocation;
and
(3) Demonstrating that the
cause for revocation does not exist at the time of the subsequent
application.
