(a) The revocation of a pharmacy permit shall permanently withdraw the authority to operate a pharmacy in New Hampshire.
(b) A subsequent permit may be obtained only by:
(1) Complying with all of the requirements of
RSA 318 and these rules regarding the original licensing of
pharmacies;
(2) Paying all
penalties assessed in connection with the cause for revocation; and
(3) By demonstrating that the cause for
revocation does not exist at the time of the subsequent application.
