Pari 1203.07 Applying for a Games of Chance Facility License.

(a) In accordance with RSA 287-D:7, I, any person or entity, other than a charitable organization or governmental subdivision, with a facility at which games of chance are held for 5 or more game dates per calendar year, shall be licensed.

(b) Each applicant for a games of chance facility license shall submit the following to the commission at least 120 days prior to the expiration of the facility's current license or games of change being held at the facility, if not currently licensed:

(1) A completed "Application for Games of Chance Facility License" form (9/2016);

(2) A description of the gaming area, in accordance with Pari 1208.05;

(3) A copy of the certificate of good standing from the department of revenue administration;

(4) Documentation and payment for background and criminal record checks submitted in accordance with Pari 1204.01 below; and

(5) Payment of the license fee of $250, pursuant to RSA 287-D:13, II.

(c) For the purpose of this section, the applicant shall be the primary owner of the property or the property manager with the authority and oversight responsibilities of the facility.
