"Employer identification number" means the 9 digit federal identification number assigned to an employer by the Internal Revenue Service. Forms to be used for the purpose of applying for a federal identification number may be obtained at any local office of the IRS, Post Office or from IRS, And over, Massachusetts. Upon receipt of this number, the employer shall forward same immediately to the agent or carrier providing coverage.
