Every licensee shall complete, sign, have the signature acknowledged by a notary public, and file form Employee Leasing Renewal Application, Rev. 07/2012, for renewal of license at least 60 days prior to expiration of its license, together with the renewal application fee of $100. The applicant for a renewal license shall supply the information required in Lab 1503.02(b)(1)-(13).
