No policy or certificate of coverage or description of coverage of blanket accident and health travel insurance shall be issued or delivered to any person or entity in this state, nor shall any application, rider or endorsement be used in connection therewith, until a copy of the policy form, description of coverage form, and premium rates have been filed with and approved by the commissioner pursuant to the provisions of RSA 415:1.
