Section Hum 207.01 - Additional Information from the Complainant

If the investigator considers the information relevant to the investigation, the investigator shall request the complainant to provide information including the following:
(a) A detailed written account of the events alleged in the complaint;
(b) A list of potential witnesses for the complainant together with:
(1) Their names, addresses, telephone numbers
and any other available identifying information about them; and
(2) A brief description of the information
each witness is expected to provide; and
(c) Any other materials relevant to the investigation of the complaint.
