During each radiographic operation the radiographer, or the other individual present, as required by He-P 4034.15, shall maintain continuous direct visual surveillance of the operation to protect against unauthorized entry into a radiation area or a high radiation area, as defined in He-P 4003.01, except at permanent radiographic installations where all entryways are locked and the requirements of He-P 4034.13 are met.
