Universal Citation: NH Admin Rules He-P 4021.19

(a) The licensee or registrant shall file a report within 5 days with the DHHS/RHS if the test for leakage or contamination required by He-P 4001.05 indicates a sealed source is leaking or contaminated.

(b) The report shall include:

(1) The equipment involved;

(2) The test results; and

(3) The corrective action taken.
