(a) To renew, an applicant shall submit all the items in (b) below and any changes to information provided to the department on the initial application.
(b) The applicant shall provide the following along with the application:
(1) A current
copy of a bulk milk hauler evaluation inspection form if it was conducted by a
SSO or DSSO from another state agency; and
(2) A check, money order, credit card
payment, or online payment for the applicable fees in accordance with
RSA
184:77.
(c) Licenses for bulk milk haulers shall be renewed provided they have had an inspection within the last 24 months. A copy of this inspection sh all be provided with the application if it has been conducted by a SSO or DSSO from an out of state agency.
(d) Each licensee wishing to renew his or her license shall comply with He-P 2707.05(a) and (b) at least 30 days prior to the expiration of the current license.
