Section He-P 2304.09 - Change in Name of a Food Establishment

(a) When a license holder intends to change the name of a food establishment, that license holder shall submit a written request to the department for a new license at least 30 days prior to the intended date of change in name.
(b) The written request shall include:
(1) The reason for requesting a new
license;
(2) The name of the food
establishment as it appears on the existing license;
(3) The name of the food establishment as the
license holder requests it to appear on the new license; and
(4) The date upon which the change in name is
intended to occur.
(c) Following receipt of the items required by (b) above, the commissioner shall issue a revised license reflecting the change in name. The establishment number and expiration date shall remain the same as it was on the immediately preceding license.
