(a) All in-home supports shall be directed and managed by the individual or the individual's representative.
(b) In-home supports shall be:
(1) Specifically tailored
to the competencies, interests, preferences, and needs of the individual and
his or her family and respectful of the cultural and ethnic beliefs,
traditions, personal values, and lifestyle of the family;
(2) Designed to facilitate, maintain, and
enhance supports from family members, friends, neighbors, child care
organizations, religious organizations, and community programs;
(3) Responsive to the individual's and
family's changing needs and choices within the limitations of federal and state
laws and rules;
(4) Specified in
the individual's service agreement, or individual family support plan
(IFSP);
(5) Provided only after the
informed consent of the individual or representative;
(6) In compliance with the rights of the
individual established under
RSA
171-A:14 and He-M 310;
(7) Supportive of the individual's or
representative's efforts to direct and manage the services to be provided;
and
(8) Delivered in collaboration
with other related support plans when applicable, and consistent with other
services provided in additional environments such as the community, school, and
work.
(c) The individual and the individual's representative shall have free choice of any willing provider meeting the qualifications of this part.
