(a) Any agency which coordinates respite care services in accordance with these rules shall have a current contract with BEAS.

(b) The contract agency shall receive inquiries regarding ADRD respite care, complete the assessment and person-centered care plan required by He-E 503.04, and other responsibilities described in this rule.

(c) The contract agency shall keep a record on each primary caregiver receiving ADRD respite care services, including:

(1) The assessment described in He-E 503.04;

(2) The person-centered service plan and budget pursuant to He-E 503.04; and

(3) Any related correspondence.
