(a) DCYF shall provide a renewal application to the resident director 4 months prior to the expiration date of the current license.

(b) The resident director shall complete a renewal application for the license within 30 days of receiving the renewal application.

(c) The resident director shall supply with or attach the following information to the renewal application:

(1) The current date;

(2) The name and address of the experiential/wilderness facility;

(3) The name and business telephone number of the resident director;

(4) An identification of residential service, population to be served, and licensed capacity;

(5) A narrative on changes made in the program during the last licensing period;

(6) The names of the board of directors and their addresses and telephone numbers;

(7) Approvals of the physical plant by the local fire department or state fire marshal's office and local public health officer; and

(8) Signature of the resident director.

(d) An inspection shall be completed at least 30 days prior to the expiration of the current license if the renewal application has been submitted to DCYF.

(e) If the application is complete and the inspection verifies compliance with He-C 6450, then renewal of the license shall be granted by DCYF.
